  4. We will acknowledge receipt and advise the next steps or direct your query to the appropriate practice team.

Checklist — information to include

  • Your full name and the email address associated with the account.
  • A short description of the issue (for example: cannot sign in; need data copy; need to correct details).
  • Relevant dates, reference numbers, or copies of correspondence.
